Your doctor has enrolled you in ClearPath's Chronic Care Management program. A dedicated care coordinator will check in with you monthly to help you stay on track with your health — right from home.

If you have two or more chronic conditions — like diabetes, high blood pressure, or heart disease — your doctor can enroll you in a special Medicare program called Chronic Care Management (CCM).
Through ClearPath Virtual Health, a trained care coordinator will call you once a month. They work closely with your doctor to make sure you're getting the support you need between visits.
You'll receive a scheduled call by phone or Zoom from your ClearPath care coordinator. It usually takes 20–30 minutes.
Together you'll go through your current medications to make sure everything is up to date and working well for you.
Your coordinator will check in on your health goals and note any symptoms, concerns, or changes since your last call.
After every call, a summary is sent to your doctor so they always know how you're doing — even between office visits.

Not comfortable with Zoom? No problem. You can always join by regular phone call. Just let your coordinator know.
ClearPath is fully HIPAA-compliant. Everything discussed during your care calls is private and protected. Only your care team and your doctor can access your health information.
